Continuing Health and Social Care
This is provision of long term care for people aged 18 years and above with physical health, mental health and disability by the NHS.
We currently supply CHC Nurses Assessors for Clinical Commissioning Groups and Commissioning Support Units covering Coventry and Warwickshire CCG among others.
We have nurses carrying out the following assessments;

4. Fast Track Assessment
5. NHS Eligibility 3 Monthly Reviews
Here in SHC Limited, we assure on effective management of caseload and clear backlogs in a most timely and cost efficient manner for all our clients by the professional and dedicated team of CHC Nurse Assessors working in partnership with us.
We have a Quality Assurance System in place to ensure that Assessments submitted by us meet and exceed the benchmark of Continuing Healthcare as stipulated in the National Framework. This we do by putting in place modalities for Senior Nurse Assessors to review and also quality-check the assessments carried out by our Nurse Assessors with the aid of robust and reliable Quality Assessment tools.
We offer specialist training for Nurses and Social Workers who are desirous of embracing this highly rewarding and flexible specialised field within the health and social care industry. The training offered by our trainers covers the following among others;
a. Introduction to NHS National Health Framework
b. Understanding Continuing Healthcare Assessment
c. Understanding Funded Nursing Contribution Assessment
d. Practical steps to CHC assessment NICU and Recommendations
e. Approach to Next of Kin and other Agencies
This training is offered by resource persons across the UK who have more than theoretical knowledge of CHC but are themselves Assessors with not less than a decade of CHC practice respectively.
